COMAH Breach Leads to £500k Fine

AW Safety Management Ltd News and PR from AW Safety Management Ltd - Published 13 January 2022 UK's Largest Oil Refinery Fined After LPG Leak
A leak of liquid petroleum gas (LPG) which went undetected for several hours was discovered at the UK’s largest oil refinery. The operator has been fined for health and safety breaches. The leak released around 15 tonnes of LPG. This went through a valve near a main roadway due to the flow having too high a pressure for the valve.

Following this, the company have now paid a £500,000 fine and a cost of £34,103 as result of not having a process in place to detect the flow in the pipe and their failure in taking necessary measures to avoid any major accident.
