A leak of liquid petroleum gas (LPG) which went undetected for several hours was discovered at the UKÂ’s largest oil refinery. The operator has been fined for health and safety breaches. The leak released around 15 tonnes of LPG. This went through a valve near a main roadway due to the flow having too high a pressure for the valve.

Following this, the company have now paid a £500,000 fine and a cost of £34,103 as result of not having a process in place to detect the flow in the pipe and their failure in taking necessary measures to avoid any major accident.
